Rental Form
Rental Requirements:
Renter must be a Major Credit Card Holder, provide a form of ID, and submit this Medical Equipment Rental Agreement.
Rental Agreement:
By submitting this agreement, the Renter acknowledges that Goodmedical rents medical equipment (including hospital beds, patient lifts, wheelchairs, knee walkers, walking aides, and nebulizers) and agrees to all outlined terms. It is important to note that the finalization of payment terms is subject to mutual agreement between Goodmedical LLC and the renter.
The medical equipment remains the property of Goodmedical and must be returned in the same condition. Goodmedical reserves the right to inspect the equipment upon demand. Unauthorized use may result in repossession without notice.
Goodmedical is not liable for any loss, damage, theft, or misplacement of the equipment. The renter assumes all risks and indemnifies Goodmedical against any related claims.
The renter is responsible for any loss, damage, or injury caused by the equipment. Reimbursement for damage or loss is required, and payment terms will be mutually agreed upon by Goodmedical LLC and the renter.
Certain restrictions apply to equipment use, including age and sobriety requirements, as well as safe operation.
The renter agrees to defend, indemnify, and hold harmless Goodmedical from any claims arising from equipment use, except in cases of fault or misconduct by Goodmedical or its employees.
All costs and expenses, including legal fees, are the responsibility of the renter.
Goodmedical is not responsible for the actions or omissions of the renter or their agents.
Prompt notification of accidents or damages is required.
By agreeing to this agreement, the renter accepts responsibility for all costs, expenses, and attorney fees incurred by Goodmedical in collecting sums due, regaining possession of the equipment, or enforcing and recovering damages or losses.
No agency or employment relationship is established through this rental agreement.
If this agreement is submitted on behalf of another person or entity, the person submitting is personally bound to all obligations.
